An entrepreneur who wants to build a new building or renovate an existing building to operate the following private health care services, shall apply to Township Supervisory Committee with the Advance Permission Application (Form-1): - 

(a) private clinic service:

(1) private general clinic service;

(2) private specialist clinic service;

(b) private hospital service:

(1) private general hospital service;

(2) private specialist hospital service;

(c) private maternity home service;

(d) private diagnostic service;

(e) private nursing home service;

(f) private mobile health care service;

(g) private health care agency;

(h) private general health care service.

The Advance Permission Application as per Rule 3 shall be applied by the respective business owner. The business owner shall: - 

(a) complete the information stated in the Advance Permission Application (Form-1) other information shall be provided prescribed by the Central Board from time to time;  

(b) The recommendation from the respective development committee or development board shall be attached to the Advance Permission Application (Form-1). 

Any person who is not a religious person, wants to operate any public health care services shall apply license for the desired public health care service with the Public Health Care Service License Application (Form-4) obtained from the Township Health Care Office and submit the application by completing the required information in the form.  

The Private Health Care Service License shall be applied with the name of the person who wants to operate it. 

The following shall be provided when applying license as per Rule 13: - 

(a) Colour Photos (4) pcs, size (2” x 1.5”) of applicant; 

(b) Colour Photos (3) pcs, each for the building (front and side) and rooms to be used for Private Health Care Service;  

(c) Original challen (evidence receipt) one each for the payment of license fee in accordance with the type of applied service deposited to the bank account of Township Health Care Office opened at Myanma Economic Bank.  

The applicant for the license as per Rule 13 while applying with the Private Health Care Service License Application (Form-4) shall attached the following prescribed forms with respect to the type of health care service by completing required information:

When applying the license for the following health care services, the advance permission issued in accordance with the Rule 8, sub-rule (b) or Rule 10, sub-rule (b) shall be attached to the application: - 

(a) private clinic service:

(1) private general clinic service;

(2) private specialist clinic service;

(b) private hospital service:

(1) private general hospital service;

(2) private specialist hospital service;

(c) private maternity home service;

(d) private diagnostic service;

(e) private nursing home service;

(f) private general health care service.

The license holder of the private health care services, desire to continue the business operation after expiration period of the license shall apply the renewal of the license at least (60) days before the expiration date by paying the designated license fee and apply with the License Renewal Application (Form-7) to the Township Supervisory Committee. For overdue application can be submitted by paying overdue fee.  

The original Challen for the payment of license renewal fee and copy of the License shall be attached when applying the license renewal as per Rule 27. 

If the license holder of Private Health Care Services desirous to transfer or leasing the private health care service to another person shall apply to the Township Supervisory Committee with the Transfer or Leasing of Private Health Care Service Application (Form-8).   

The Private Health Care Services shall accept the inspection made by the respective Supervisory Committee from time to time.
